Targeted Case Management


  • Serves youth both male and female, 12-20 years old
  • Provides intake, assessment, case planning through identified goals, coordination of services, advocacy, monitoring and referrals

Eligibility


  • MaineCare insurance carrier
  • Diagnosed behavioral health disorder (long-term case management)

Examples of clients that we can serve (but not limited to):


  • Youth with a diagnosis, on probation or conditions of release with unmet service needs (example: a 17 year old young woman not enrolled in a schooling program and in need of substance abuse counseling and individual therapy for anxiety and depression)
  • Youth enrolled in the Alternatives to Detention Program (example: a 16 year old young man, high school student, with conduct disorder in need of pro-social activities, community monitoring, and MST services, graduating ATD program in need of step-down support)
  • Youth enrolled in YBA or Service Works Programs that have a diagnosis of a behavioral health disorder (examples: 15 year old young man in need of 50 hours of community service owed to the courts and an advocate at his next IEP, 17 year old young woman enrolled in YBA diagnosed with depression and in need of pre-natal care)
  • Youth with a behavioral health disorder diagnosis i.e. Major Depressive Disorder, Bi-polar Disorder, ADHD, an Anxiety Disorder in need extra support (example: 18 year old single mother with bi-polar disorder in need of housing and TANF services)
